The permit that refuses to issue

A permit to work, or a lifting plan, that names a person whose competency has lapsed — or calls up a shackle past its thorough examination — is refused at issue, with the reason on the screen. That single rule is what turns two registers into a safety system.

One lifecycle for every safety document

Method statement, JSA, lifting plan, transport plan, cargo-securing calculation, risk assessment and emergency plan move through the same route: draft, review, approve, issue at Rev A, brief and acknowledge by name, supersede. Only one revision is ever current, and an acknowledgement of a superseded one is worth nothing.

Engineering, on the page

The lifting plan computes the gross load, the tension in each sling leg at the angle it actually sits at, the utilisation of every component against its own WLL, the crane's chart utilisation and the pressure under the outrigger mats. The cargo-securing plan shows the EN 12195-1 calculation term by term — friction, acceleration coefficients, the securing balance in each direction — not a picture with a number under it.

Near misses count, because they are free

A near miss is the same record as a lost-time injury with a different outcome, so it gets the same five-why investigation, the same contributing factors and the same corrective actions. LTIFR, TRIR, the severity rate and the days since the last LTI come out of that register, computed on the exposure hours you entered rather than a guess.

Every shackle has a date

Shackles, slings, chains, hooks, spreaders and beams, eye bolts, turnbuckles, lashing straps, chain blocks, jacks, skates, air bags and dunnage — each with its WLL, its proof-load certificate, its six-monthly thorough examination, its colour-code quarter and its custodian. Fail an examination and the item quarantines itself and suspends every live permit that called it up.

Nothing closes because somebody said so

An incident cannot close with a corrective action still unverified. An action cannot close because the person who did the work says it is done — somebody else has to check that it worked, and the register records who, when, and whether it actually did.
